If your circumstances change or you need urgent help

If your needs or circumstances change, contact your practitioner. They will review your situation and may update your support plan.

Your support plan includes contingency arrangements for emergencies. If these fail and you cannot access the support you planned, contact the council for urgent help.

If you need support outside normal office hours, call the Emergency Duty Team.
