Who can use the support tool
Our support tool is for adults over 18 and living in Cumberland.
The support tool can help you if you are:
- living with a disability
- managing a health condition
- finding things difficult such as moving around your home or getting dressed
- caring for someone
If you are caring for someone, the tool can help you find support to make caring easier and look after your own wellbeing. The tool may also suggest starting a care needs assessment, which will ask more detailed questions to explore the support you may need.
