Personal licence

You must let us know of any change in circumstances. This includes:

• a change of name or address
• any conviction for a relevant or foreign offence
• if you require a duplicate copy of a licence following loss or theft

What you need to provide

You must provide proof of your home address such as a:

• bank statement
• utility bill
• Council Tax bill
• driving licence

Surrender a licence

To surrender your personal licence, you must notify the licensing authority that issued the original licence.

When surrendering a licence to us, post your original licence (both ID card and paper copy).
