- the application form has been submitted
- all required evidence has been submitted
- full payment has been made

To apply for a dual taxi and private hire driver licence, complete the application form.
Supporting evidence and documentation
You must submit your:
- valid DVLA photocard driving licence
- DVLA check code - view or share your driving licence information on GOV.UK
- valid UK passport or right to work share code - prove your right to work to an employer on GOV.UK
- DBS certificate number or Enhanced Disclosure and Barring Service (DBS)
- completed medical form, if one is due
- tax check code - complete a tax check for a taxi, private hire or scrap metal licence on GOV.UK
- 2 passport size photographs - the applicants full name should be written on the back, in block capitals
- Safeguarding Awareness certificate
